Care Co-ordinator

Joanne Williamson is the Care Coordinator for the surgery and works closely with the District Nursing Team and Community Matron.

Joanne can help you to get the support you need to remain independent in your own home for as long as possible. If you are a family member or carer of someone and you also feel you need some extra help, support or information I may be able to you too.
I am a point of liaison for patients and health and social care professionals and can help or signpost patients and carers to relevant services. Please contact the reception team for a referral to Joanne.

Medicines Order Line

The NHS Prescription Medicines Order Line service is the easy way for you to order your repeat prescription. All it takes is a simple phone call that you can make from the comfort of your own home. Your prescription can then be sent electronically to a nominated pharmacy of your choice or collected from your GP practice if preferred.

Community Matron

Community Matron, Amanda Fenton works closely with our Care Homes to support pro-active management of long term and complicated health conditions of the Frail and Elderly patients, as part of this role Amanda works in partnership with the GPs at the surgery. Amanda also provides care to housebound patients in managing their long term or complicated health conditions, enabling and educating patients to help them live with, and manage their disease.
